The Prepare Team - City of Edinburgh Council Children and Families Department Partner organisations: NHS Lothian PrePare is a multi-disciplinary team co-located within a child and family centre in the city of Edinburgh. In Edinburgh many babies each year are born with the effects of substances to which they are exposed before birth. Neonatal Abstinence Syndrome and Foetal Alcohol Syndrome have many devastating and complicated effects on children after birth and throughout their lives. The dynamic, creative, imaginative and enthusiastic team have worked with 172 women since July 2006 aiming to maximise the health and wellbeing of pregnant women who have substance misuse difficulties. The aim is to support children at the earliest point, to give them the best chance when beginning their lives. Barnardo's Works - Barnardo's Scotland Partner organisations: Scottish and Southern Energy (SSE) Barnardo's Works is an innovative and successful programme that allows disadvantaged young people to achieve independent employment. Since 2007 Barnardo's Scotland has worked with Scottish and Southern Energy to offer jobs in a wider range of industry sectors, which has resulted in the sharing of skills, knowledge and resources from two different agencies, each playing to their strength. Aberdeen Social Enterprises - Turning Point Scotland Aberdeen Social Enterprises (a Turning Point Scotland service) team is fully committed to promoting the independence of individuals who are disadvantaged in the labour market through the provision of training, work experience and employment. Dedicated to reducing the enterprise dependency on funding, staff go the extra mile to enhance business opportunities and save money. The team of nine support up to 50 people who use services at a time within four distinct enterprises - a craft workshop, a framing workshop, gift shop and café. Putting personalisation into practice - Care Accolades 2010 [04:31]
Meadowview - A Partnership in Practice in Transition to Adult Services - North Ayrshire Council Meadowview is a unique service, home to four young people who have profound and multiple learning disabilities and complex health needs. Friends at the same residential school, where trained nurses and therapists met their complex needs had to change to adult services once they turned 18. North Ayrshire Council, the local Community Learning and Disability Team and the provider formed a partnership to engage with the young people and their families. This resulted in the four friends sharing a home, Meadowview, just like other young people. Getting a life, not a service - ENABLE Scotland 'Getting a Life Not a Service' is the theme of ENABLE Scotland's strategy that sets out to work in partnership with local authorities, people who have learning disabilities and their families to achieve personalised support. The strategy has empowered 600 people who have learning disabilities and their families to participate in debate and discussion on personalisation through conferences and events. As part of the strategy, we worked with the Scottish Government pilot in East Glasgow CHCP. Working with six willing individuals to develop support plans with clear outcomes, and to use their individual budgets to achieve those outcomes.
